**Form 8** is the primary form used for updating voter details, shifting constituency listings, or ordering a replacement Voter ID card.
Note: Address shifting can be requested within the same constituency or across different constituencies within India.

Categories of Form 8 Applications
Form 8 handles four distinct voter service workflows. Choosing the correct category ensures smooth processing:
- 1. Shifting of Residence: Select this if you have permanently relocated to a new address. It updates your constituency allocation and deletes the registration at your old house. You must submit address proof of your new residence.
- 2. Correction of Entries: Select this to modify spelling mistakes or values in the existing voter roll. Fields that can be modified are: Name, Gender, Age/Date of Birth, Relation Type (Father/Husband), Relation Name, Photo, Address, or Mobile Number.
- 3. Replacement EPIC: Use this option if your Voter ID card is lost, damaged, destroyed, or mutilated. You can request a replacement card. If the card is lost, you must upload a copy of a police FIR/complaint. If damaged, no FIR is needed.
- 4. PwD Marking: Enables differently-abled voters to register their category of disability (locomotive, visual, speech, etc.) to receive assistance or transport support on polling days.
Required Documents for Corrections
Supporting files must be uploaded according to the type of correction requested:
- For Name Change: Official Gazette notification, marriage certificate, or government document showing name change.
- For DOB / Age Correction: Aadhaar Card, Passport, birth certificate, or Class 10 school leaving certificate.
- For Address Change: Recent electricity bill, water bill, gas invoice, bank statement, or registered lease deed.
- For Photo Update: A high-quality digital passport size photo.
- For PwD Marking: Medical disability certificate issued by a government board.
